Inquiry should also be specifically directed at identification of risk factors for OA including prior vasectomy, prior inguinal or scrotal surgery and history of epididymitis or sexually transmitted infections. The performance characteristics of each sperm acquisition procedure are presented in Table 1. If they do not wish to undergo a vasectomy reversal, or if they have had a failed reversal, PESA is a good way to help them reproduce. Only a very small incision need be made under local anesthesia to be able to access the epididymis. In contrast, normal volume, normal pH, fructose-positive azoospermia suggests either epididymal or vasal obstruction or NOA. In most cases, motile sperm may be found in the proximal portions of the epididymis and even in the testes of men with OA.
